Required Documents

Before you start your application, make sure you have:

• Statements of achievement (or other confirmation of successful completion) of required competencies submitted to the HCRA and/or notification from the HCRA that any required equivalencies or exemptions have been granted
• Criminal Record and Judicial Matters check results

In addition, you may require the following documents to successfully complete your application. This list applies to the primary contact and any principals, officers, directors or interested persons in your organization.

• Date of Birth
• Driver’s Licence Number
